Progress To Degree Forms

The Progress to Degree forms (PTD) enables the university to track and collect vital information about students’ progress to degree.

The PTD form is a fillable PDF form. To view and complete these files you can download the latest version of Adobe Reader from http://get.adobe.com/reader/. You can also download a complete PTD form.

To process any of the forms, please do the following:
1) Select the appropriate function,
2) Complete the required information
3) Print the form
4) Obtain necessary signatures
5) Follow the workflow on each form and forward to the appropriate office(s).

Change in Plan
This form is to change the degree type within the same program or change the course of study within the same department.

Change in Academic Status
Change in academic status includes good standing, probation, continued probation, subject to dismissal, and dismissal.

Permit to Re-enroll-Domestic Student
Permit to re-enroll is used when a student wants to re-enroll in a graduate program after not enrolling the previous semester(s).

Permit to Re-enroll-International Student
Permit to re-enroll is used when a student wants to re-enroll in a graduate program after not enrolling the previous semester(s).

Time Limit Extension Request
This form provides a student an extension on the time limit for a masters or doctoral degree

Leave of Absence Request
Students may request and departments may grant a leave of absence from a graduate program. Time used during a leave of absence does not count towards time to degree limit. Use this form to mark the beginning of a leave, and then again to end the leave.

Master’s Degree Credit Transfer
Transfer of courses toward KU Master's degree

Completion of Master’s or Specialist Exam
To be completed when student completes master's or education specialist (EdS) degree.

Completion of Comprehensive Oral Exam for Doctorate
Completed when the student passes the comprehensive oral exam.

Completion of Final Oral Exam for Doctorate
Completed when a student successful completes the final oral examination for the doctoral degree

Completion of Graduate Certificate
To be submitted when a student completes the requirements for a graduate certificate.

Miscellaneous Functions
This form can be used to track miscellaneous internal items.  Some examples may include: reduction in credit hours, petitions, etc.
